OSHA Proposes Fine In Workplace Maiming

A food processor in Ohio is facing a potential six-figure fine from the Occupational Safety and Health Administration for a 2020 accident that resulted in partial amputation of a worker’s arm.

The accident occurred in the Lima, Ohio, facility of BEF (Bob Evans Farms) Foods Inc. A 39-year-old worker was cleaning an auger used to rice potatoes when it activated, causing multiple lacerations that resulted in partial amputation of her left arm. According to an OSHA news release, other workers had to cut apart the equipment to free the victim.

OSHA determined after an investigation that BEF had failed to practice proper lockout/tagout procedures or train their employees in them. It is proposing a total penalty of $136,532. The agency cited similar violations in 2016 at the same facility.
